2011-2012 Youth Hockey Program Fees
Every MAYHA hockey player and coach must be a member of USA Hockey in order to play or practice in the hockey leagues at the Glacier Ice Rink. To register with USA Hockey for the 2011-2012 season, please register by clicking the USA Hockey link below and bring your confirmation page to sign-ups.
At registration you will need your USA Hockey Number, a copy of your birth certificate if not already on file and payment(checkbook, visa, or matercard).

Registration Packet : includes registration form, fundraising form, MAYHA Player Code of Conduct, and MAYHA Parent Code of Conduct - All required at registration.

Coaches Code of Conduct Scholarship Form
Credit Card Information Form for Payment Plan
If you can't make it to GIR to register, download the registration forms, fill them out and drop off at the GIR Office or Mail to:

MUST HAVE POSTMARK OF SEPT. 15 or OCT.15 TO RECEIVE THE BEFORE FEES! Your child will NOT be allowed on the ice until all required registration forms, fees or payments are current and Fundraising Checks to hold are turned in.
*Registration is processed on a first-come, first-served basis until each league is filled.
